How to fill out the Inhabitant tax declaration form

You need to declare to the municipality that you were not earning any money during last year. Please refer to this example for how to fill out the form.
After filling out the form, visit Onna village hall lobby on the ground floor between 8:50 and 11:30 without prior reservation to submit the form.
Show the following line to the officer to indicate that you are there to submit the form: 「村・県民税の申告書の提出に来ました」

What you need to do:

Contact your employer and ask if they have submitted 'the certification of remuneration paid(給与支払報告証明書)' to your municipality last year.

>>If they have, you don't need to do anything with the form you received.

>>If they haven't, prepare a tax withholding slip(源泉徴収票) your employer issued you back in December and refer to this example when filling out the form, or contact the Resource Centre to seek help in filling out the form together.
