参加同意書・免責事項確認書はこちらからダウンロード可能です。内容を確認いただき、承諾の上、ボックスをチェック及び記名をお願いします。
尚、参加当日に参加同意書の提出をお願いします。必ず保護者の直筆の署名が必要ですので、忘れずにご対応頂けますようお願い申し上げます。

Please download the Participation Agreement and Consent Form from here. Please check the box and sign the form after confirming the contents and agreeing to the terms of the agreement.
Please submit the Consent Form on the day of the workshop. The parent/guardian must sign the form in his/her own handwriting.
