What to bring...

Step1: Resident Registration/住民登録

  1. Go to counter #1 and ask staff  for the forms, fill out as sample and submit them.
    a) Notification form of address change/住民異動届 (sample)
    b) Application form for Certificate of Residence/住民表交付申請書 (sample)

    *If you have residency other than Onna village in Japan, "Certificate of Deregistration/転出証明書" from your previous residential municipality is requried.
     
  2. Obtain Certificate of Regidence/住民票.
    *Make sure your "My number(Japanese social security number)" is written.

Step2: National Pension Registration/国民年金

Ask staff for the forms at counter #1, fill out as sample and submit them. 

a) Application form for joining National Pension/国民年金被保険者関係届書 (sample) 
b) Ticket of acceptance of application form for National Pension/年金相談受付票 (sample) 
 
If you apply for the National Pension Exemption... 

  • General Exemption [Download / sample] (PhD/SRS)  
    (For PhD) You will need to apply General Exemption first for the period between entry date and your enrollment start date. 

  • Student exemption [Download / sample] (PhD)   
    AFTER your enrollment starts, you can fill out the form and submit it and Certificate of Registration to SAS during enrollment period/week, SAS will send it to the pension office. After enrollment week, we will help you fill out the form, but you will have to submit them to the pension office/municipal office by yourself. 

* Certificate of Registration / 在籍証明書 =>Please apply from HERE 
*It will take 3 business days to issue certificate. 
  Please come pick up at GS counter only after you receive negative result of 1st PCR test.  

e.g. Arrived in Japan in Aug 15, enrolled in Sep 1. 
 >General exemption for Aug 15-31 
>Student exemption for after Sep 1

 

Step3: National Health Insurance Registration/国民健康保険

**Only PhD/Special Research Student/Science Communication Fellow**

  1. Go to counter #3 and ask staff for the form, fill out as sample and submit it.
    a) National health insurance registration form/国民健康保険資格取得届 (sample)
     
  2. Obtain a national health insurance card

    *You can pay the national health insurance fee at municipal office on the day, or pay by payslips at convenience store or bank later (by due date).
    *You can set up the automatic payment from your bank account if you have a Japanese bank account.

Step4: Submit Documents to the Student Affairs Section

  1. Certificate of Residence/住民票
  2. Residence Card

Step1: Resident De-registration/住民登録抹消

  1. Go to counter #1 and ask staff  for the forms, fill out as sample and submit them.
    a) Notification form of address change/住民異動届 (sample)

Step2: National Pension De-registration/国民年金脱退

  1. Ask staff  for the forms at counter #1, fill out as sample and submit them.
    a) Application form for joining National Pension/国民年金被保険者関係届書 (sample)
    b) Ticket of acceptance of application form for National Pension/年金相談受付票 (sample)

Step3: National Health Insurance De-registration/国民健康保険脱退

  1. Go to counter #3 and ask staff for the form, fill out as sample and submit it.
    a) National health insurance registration form/国保資格喪失届 (sample)

    *If there is missed payment, you need to pay before de-registeration.

When you move into a new place, your registered address at the Municipal office should be changed accordingly.

1-A. Move within Onna village.

Please go to the Onna village office to change your address within 14 days after you have moved.

After your new address has been registered, please obtain a Certificate of Residence (住民票) and bring a copy or submit via GigaCC [Instructions] along with your updated Residence card (front & back) with the Student Affairs Section.

1-B. Move to other Village/Town/City

(1) De-register at Onna village office

They will accept your application 14 days before your moving-out date.
A Notification of moving out(転出届)will be issued.

(2) Re-register at the Municipal office where you are moving in.

You need to be registered again within 14 days after you have moved in. After your new address has been registered, please obtain a Certificate of Residence (住民票) and bring a copy or submit via GigaCC [Instructions] along with your updated Residence card (front & back) to the Student Affairs Section.

Also, please make sure to obtain a new National Health Insurance. If you prefer, please re-set the bank auto deduction.

2. After your address has been changed

  1. Please submit a copy of the Certificate of Residence(住民票) and your updated Residence card (front & back) to the Student Affairs Section via GigaCC. [Instructions]
  2. Once you have sent them by GigaCC, please let us know by email.

* If you have Japanese nationality, just send us the Certificate of Residence.

Afterward, please apply for a mail transfer service at the Post office, and also change the address of your Bank account, Driver's license, Car registration, Credit card, and so on.

3. Commuting Allowance

In principle, students who live outside the campus and need to commute are expected to use the OIST shuttle bus. However, a commuting allowance may be paid to students who live outside of campus but cannot use the OIST shuttle bus for reasons deemed valid by the Dean of the Graduate School. (Student Support Policies >1.4 Commuting Allowance) 

To receive a commuting allowance, you must submit a "commuting notification form" (see below) and an official document that we can confirm your new address (Certificate of Residence issued after address change at the municipal office/Residence card etc.). Please note that the allowance will be paid from the month in which these documents are submitted, and no retroactive payments will be made for months prior to that month. If there are any special reasons for the late submission, please contact GS financial team in advance.

 
  • Commuting Notification 
    To download the form, please visit forms.
    You must return the bus pass to the Graduate School.

    *If you haven’t received your residence card yet, you will receive it at Onna municipal office when you register.   

  • Certificate of Residence (with My Number) 

  • Certificate of Entry / 入学予定証明書 (for who have not started enrollment yet) 
    =>SAS will prepare for you.
    Certificate of Registration /在籍証明書 (for who have already started enrollment) 
    =>Please apply from HERE 
    *It will take 3 business days to issue certificate. 
    Please come pick up at GS counter only after you receive negative result of 1st PCR test.  

  • Some money (yen) to deposit into your bank account
