Municipal office registration procedure

*Please note that the access to the municipal office and forms will be different if you live outside of Onna village

*Student who doesn't receive residence card at the airport, you will receive one in few weeks after registration at municipal office. Please submit it to via "GiGaCC" [For PhD / For Non-PhD Students] (Instruction)

 

Belongings

Take along CHECKLIST (Registration) with you so that administratives at the office can easily know the purpose of your visit.

*Make sure to write "contact information" and "contact person" on CHECK LIST.

Japanese Non-Japanese
  • ID with photo (Driver’s license/ My number card/ etc…)
  • Inkan seal
  • Pension book or Pension card 
  • (For PhD students/SRS) 200 yen for issuance Certificate of Residence

(If applicable)

  • My number card
  • Certificate of De-registration
  • Certificate of Health Insurance Disqualification
  • Passport
  • Residence Card
  • (For PhD students/SRS) 300 yen for issuance Certificate of Residence

(If applicable)

  • Inkan seal
  • Pension book or Pension card
  • My number card
  • (For spouse) Both original and English translation of Marriage Certificate
  • (For children) Both original and English translation of Birth Certificate
 

Access

  1. Take OIST Shutle Bus (Seragaki Line / But stop: Onna Village Office)
  2. Take local bus (Map) *Bus comes every minutes.
    Route Finder: Bus Navi Okinawa
    Bus number: Okinawa bus/沖縄バス [#20] or Ryukyu bus/琉球バス [#120]
    Bus stop names:
    Departure: Daigakuin Daigaku Mae/大学院大学前
    Destination: Onna-son Yakuba Mae/恩納村役場前

 

Step1: Resident Registration

  1. Please get the form from GS counter/municipal office and fill out as sample below.
  2. Submit to counter #1 at municipal office. (*If you have residency other than Onna village in Japan,
          "Certificate of Deregistration/転出証明書" from your previous residential municipality is requried.)
  3. (For PhD/SRS) Make sure your "My Number (Japanese social security number) " is written in Certificate of Residence.

  • Application form for Certificate of Residence/住民票交付申請書 (SAMPLE)

 

Step2: National Pension Registration

  1. Please get the form from GS counter/municipal office and fill out as sample below.
  2. Submit to counter #1 at municipal office.
  • Application form for National Pension/国民年金被保険者関係届書 (SAMPLE) 
  • Ticket of acceptance of application form for National Pension/年金相談受付票 (SAMPLE)

If you apply for National Pension Exemption... 

  • General Exemption [SAMPLE] (PhD/SRS/VRS/RI)  
    (For PhD) You will need to apply General Exemption first for the period between entry date and your enrollment start date.
     e.g. Arrived in Japan in Aug 15, enrolled in Sep 1. 
     >General exemption for Aug 15-31 
    >Student exemption for after Sep 1

  • Student Exemption [SAMPLE] (PhD)   
    AFTER your enrollment starts, you can start applying for Student Exemption.

 

Step3: National Health Insurance Registration

  1. Please get the form from GS counter/municipal office and fill out as sample below.
  2. Submit to counter #3 at municipal office.
  3. Obtain a National health insurance card.
    *You can pay the national health insurance fee at municipal office on the day, or pay by payslips at convenience store or bank later (by due date).
    *You can set up the automatic payment from your bank account if you have a Japanese bank account.

Step4: Submit documents to Relocation Support Team

Please submit documents below to Relocation Support Team or send them via [For PhD / For NON-PhD Students] (Instruction)

~PhD/SRS~

  • Certificate of Residence                                                                                                               

         *Make sure your "My Number (Japanese social security number) " is written in the Certificate.

  • Residence card (both front/back)
  • National Health Insurance Card

~VRS/RI~

  • Residence card (For those who will stay in Japan more than 90days)
  • National Health Insurance Card
  • Passport

*When your travel arrangement was supported by RUA (Research Unit Administrator), please share above information with your RUA.

Step5: Register address for mailing service

To receive postal service, you need to submit a "The Change of Address Notice". Please print below document out and fill in new address information, make sure to write all the residence name in your house. After filling in all the information, put "The Change of Address Notice" in a postbox. No stamp necessary.
