Welcome to OIST Science Challenge! 

Please fill out the form below regarding your transportation and accommodation during the workshop, as well as activities preferences. Make sure that you have read the OIST Travel Rules before filling out this survey.

Please fill out the survey and submit it no later than Friday, FEBRUARY 8th, 11:59pm Japan time (GMT+9). Also, do not forget to attach the scanned copy of your passport and reply to the email address below.

Personal information
Please write the name of the airport in English. Note: OIST will buy your plane ticket to Okinawa, please don't purchase one yourself.
You will be arriving to OIST on Saturday, March 9th. OIST will provide your transportation and accommodation until the morning of Saturday, March 16th. If you decide to stay longer, you must provide your own local transportation and accommodation.

On Sunday, March 11th, we will organize an Cutural Excursion day for you to visit Nago Ocean Expo Park . There are various attractions within the park for you to choose from, however please understand that entrance tickets and lunch will be on your own. We will provide breakfast, dinner and transportation for the day.

Disclaimer: OIST makes every attempt to identify ingredients that may cause allergic reactions for those with food allergies. Every effort is made to instruct our food production vendors on the severity of food allergies. However, there is always a risk of contamination. There is also a possibility that manufacturers of the commercial foods we provide could change the formulation at any time, without notice. Participants concerned with food allergies need to be aware of this risk.
The photos will be used to record the event and shared with other participants. Some of the photos may also be used for future publications or advertisement.
We would like to communicate with you through a Facebook group in order to send you information and share photos of the workshop.
We would like to use Dropbox as a mean to share documents, presentations, and information. If you don't have an account yet, please create one.
