Student Evaluation of Teaching

All students should complete the survey before the end of term, before grades are allocated.  Faculty only receive the survey results after they have entered their final grades. Please be frank: your comments and answers cannot affect your grade!  Your replies will help to raise the standard of future teaching.

Faculty receive the averaged numerical values (1-5 scale, +/- SD) for answers in response to questions 1-16.  Faculty receive frequency histograms of answers to questions 17-19. Faculty receive all comments you provide here, as a single summary sheet, not associated with other identifying scores or information.

Please answer all numbered questions; the comments box at the end is optional but your qualitative feedback is helpful to faculty and to GS to improve.

All replies are confidential and non-identifying (no names or other details are recorded.)

You may also provide separate comments directly to the GS if you wish.  Please contact gs-cp@oist.jp, in complete confidence.

Form updated November 2022.

Which course are you evaluating? Make a separate evaluation for each course you took.
You should select "multiple" (last entry) for multi-teacher courses

If you have no particular opinion or reaction, then please use the neutral response, number 3.
