[Child allowance] How to submit "the present status report" - Uruma
The "Present status report" is a report which is to be submitted to the municipality every year in June to confirm the household's current childcare and income status in order to provide the household with child allowance. In case the report is not submitted by the due date, the allowance to be provided in October may be delayed. For Uruma residents, the letter comes in a pink envelope.
Uruma city office advises people to submit the report by post to relieve congestion at the city hall to prevent the spread of COVID-19. Please fill out the form, put it along with other required documents in the return envelope enclosed, and post it by Wed, 30th June(no need for a stamp). The instruction on how to fill out the report is available here.
Please contact the Resource Center if you need assistance in filling out the form since the form must be filled out in Japanese.
If you are submitting the report by post
- Documents to be enclosed:
- Filled-out and signed "Present status report" (the report must be filled in Japanese)
- A photocopy of the recipient's health insurance card(recipient is either of the parents/guardians)
* Please contact the Resource Center if the recipient is living separately from the child as you may be required to submit "the notice of separate custody " on top of the documents above.
If you are submitting the report at the city hall in person
- Where to submit: Children and Families Division located on the 2nd floor of Uruma city hall (eastern building)
- Period of submission: Thu, 17th June - Wed, 30th June
- Reception time: From 8:30 - 17:15 (except Sat, Sun, bank holidays, and Okinawa Memorial Day 23rd June)
- What to bring:
- The present status report (needs to be filled in Japanese and signed in advance)
- A photocopy of the recipient's health insurance card (there's no need to bring if you are enrolled in the national health insurance plan)
*In case the recipient is living separately from the child the applicant will be required to fill in the "Notice of separated custody". Please contact the Resource Center to find out what to do.
**Please contact the Resource Center if the child was registered in a different municipality as of 1st June 2021.
