[Child allowance] How to submit "the present status report" - Onna

The "Present status report" is a report which is to be submitted to the municipality every year in June to confirm the household's current childcare and income status in order to provide the household with a child allowance. In case the report is not submitted by the due date, the allowance to be provided in October may be delayed.

If the child becomes 15 years old or older as of April 1st 2021, there is no need to submit the report.

This year, due to the outbreak of COVID-19, Onna village is accepting the submission via post if you want to refrain from visiting their office. Please print out the form from here, fill it out and send it to the address indicated below by Wed, 30th June. The instruction on how to fill out the report is available here.

Please contact the Resource Center if the recipient is living separately from the child or if you need assistance in filling out the form.

If you are submitting the report by post

  • Documents to be enclosed:
  1. Filled-out and signed "Present status report"
  2. A photocopy of the recipient's health insurance card(recipient is either of the parents/guardians)

Print out the above address→cut it out→glue it on the envelope→buy an 84JPY stamp at Jimmy's and stick it on the envelope→put the envelope in the red mailbox in front of Jimmy's

If you are submitting the report at the village hall in person

  1. The letter received from the municipality (no need to fill out the report in advance)
  2. A photocopy of the recipient's health insurance card (there's no need to bring if you are enrolled in the national health insurance plan)
  3. Inkan(personal seal) or signature if inkan is not available

*In case the recipient is living separately from the child the applicant will be required to fill in the "Notice of separated custody". The form is available at the municipal office

**If the child was registered in a different municipality as of 1st June 2021, the applicant must present the child's "my number" information
