Application Form for Use of Research Facilities ｜研究設備利用申込書
Please read before entering this form:
1. Who is required to submit this form: A person or representative of a group applying for Use of OIST Research Facilities.
2. Those who apply with above 1 needs to attach a letter of agreement (form 1) with a seal or signature of your supervisor who has management responsibilities over your application (e.g. President, Head of Research Center, Head of Department) , as a PDF format.
3. If applying in group, what members (excluding representative) are required to submit:
All members (excluding representative) are required to submit User Pledge (for Members) online.
Application ID is required to submit User Pledge (for Members), which will be notified to representative (the person who submitted this form) in automatic confirmation email, after submission.
4. Cautions: Your entry can NOT be saved before submission.
(1) Please scroll this page and confirm information necessary.
(2) We recommend to save your writing, such as "Summary of Research", in your computer before entry.
5. Language: Please enter in English, unless specified otherwise.
There are some fields for Japanese speakers to enter in Japanese. (Names, etc.)
本申込書を入力される前に、お読みください。
1. 本申請書の提出が必要となる方：OISTの研究機器利用を申し込まれる方、またはグループで申し込まれる場合は、その代表者
2. 上記１に該当する方は、本申請に関する責任を有する管理者（例：学長、研究センター長、学部長）の印 がある承諾書（様式1）をPDF 形式に変換し、添付して下さい。
3. クループで申請される場合、代表者以外のメンバーの方に提出いただくもの：
代表者以外の全てのメンバーの方には、オンラインにて利用者誓約書（メンバー用）の提出が必要です。
利用者誓約書（メンバー用）提出には、申込書IDが必要です。
申込書IDは、利用申込書提出後に、代表者（本申込書を提出された方）にメールにてお知らせします。
4. 注意： 入力内容は、途中保存できません。
(1) 本ページをスクロールし、入力に必要な情報をご確認ください。
(2) 「研究概要」等は、入力前に記入内容をご自身のコンピューターに保存されることを推奨します。
5. 入力言語： 英語 （「日本語で入力」と明記された項目を除く）
日本語話者の方には、氏名、ご所属名等を日本語で記入いただく項目があります。
